Skilled ADON Full Time

Supervised by:  Director of Nursing

Purpose of the Position:

A unique and versatile position in the Skilled Nursing/Terrace View Neighborhood of the Friendship Haven campus. The ADON will be responsible for the household operating smoothly, promoting clinical, and quality excellence, social needs and fiduciary responsibility to enhance the resident’s wellbeing and ensuring a high level of customer service and satisfaction.  The ADON will collaborate with the team, residents and employees to determine what clinical and support services the neighborhood needs to live with faith, passion and purpose.

Essential Duties

  • Responsible for coordinating the care needs for the skilled nursing residents from admission to discharge.

  • Complete admission/discharge paperwork for each resident, making necessary discharge referrals, identifying and implementing education/teaching in preparation for discharge and other miscellaneous duties relating to discharge needs.

  • Provides education and performs care reviews to neighborhood team members.

  • Assists the neighborhood with residents/family/staff concerns, as needed.

  • Assists neighborhood in day-to-day nursing staff vacancies, as needed and able.

  • Assists in other various opportunities, as able and necessary, to meet the care needs of the skilled care residents.

  • Participates in the on-call rotation for in the clinical leadership team.

  • Provide Social Services such as one-on-one counseling, depression screening, reassurance and mental health referrals, as needed.

  • Explain and provide required Medicare notices per federal and state regulations to skilled residents.

  • Work with various outside Agencies, as needed.

  • Develop and update educational materials that can be utilized for teaching and provided to the Skilled resident upon discharge.

  • Serves on Friendship Haven Committees, as needed.

  • Supervises Terrace View nurses and Lead, other members of the IDT as delegated.

  • Assists fellow Co-ADON with other area of Simpson Health Center as needed.

  • Other duties as assigned.

Care Plan and Documentation Functions

  • Facilitates and completes all clinical portions of the care plan and skilled MDSs.

  • Participates in care plan conferences for all household residents and communicates all conference results with team members. 

  • Implements nursing/clinical interventions based on identified goals and updates plan accordingly.

  • Responsible for the investigating and reporting all incidents and accidents .      

  • Audit clinical documentation to ensure proper entries by team members and checking for completeness, quality assurance documentation and maintenance.

  • All other duties as assigned.

Knowledge, Skills and Abilities

  • Proficiency in computer usage and related applications.

Minimum Requirements

  • Education:  Licensed RN, a background in Human Services is beneficial but not required.

  • Must be CPR certified.

  • Long Term Care experience preferred, but not required.

  • MDS certified beneficial, but not required.

Physical Demands

  • Sits, stands, bends, lifts and moves intermittently during work hours.

  • Willing to work beyond normal working hours, and on weekends and holidays when necessary.

Mental Demands

  • Subject to frequent interruptions.

  • May be subject to hostile and emotionally upset residents, families and visitors.
