Journeys Nurse Leader

Purpose of the Position:

The Journey's Nurse Leader would be responsible for creating a home where residents living with dementia are valued, understood, and cared for with dignity. This leadership position supports the entire team within our dementia friendly household, ensuring exceptional, person-centered care that supports each resident's individuality, comfort and wellbeing. This clinical mentor will foster teamwork, excellence, collaboration and compassion in all their interactions, assisting all other Journey's employees with knowledge and expertise of dementia care strategies and techniques to help Journeys resident live with faith, passion, and purpose.

 

 

Household Leadership Functions:

•        Provide leadership and vision to the household by modeling professionalism, customer service, collaboration, flexibility and acts as a coach to keep processes resident centered.

•        Contributes to and consults on household personnel management: hiring, training and mentoring.

•        Works cooperatively and respectfully with residents, co-workers, families, visitors and

volunteers to create a warm and positive environment in the home.

•        Achieve the financial objectives of the community by contributing to and accounting for the annual budgets.

•        Build relationships with residents, families and guests and plays a key role in managing resident and family complaints.

•        Maintain a safe, comfortable functional environment by assessing the household for sanitation and potential hazards, implementing practices utilizing disaster preparedness and emergent care, investigating and reporting all incidents and accidents, observing the team for safe work habits and addressing observations.

•        Encourages a climate of continued learning and personal growth for all.

•        Facilitates each household’s ability to plan and meet the clinical needs of residents and the household.

•        Ensure household meetings (resident, employee and neighborhood) take place appropriately, engaging all residents and employees. 

•        On call rotation with SHC Supervisor Nursing team, as assigned.

•        Attends or assures the household is represented on all appropriate councils and teams of the community.

•        Encourages extended family participation.

•        Works with SHC team to implement and improve systems.

 

 

Care Plan and Documentation Functions:

•        Facilitates clinical portions of care plan.

•        Participates in care plan conferences for household residents and communicates all conference results with team members. 

•        Implements nursing/clinical interventions based on identified goals and updates plan accordingly.

•        Audit clinical documentation to ensure proper entries by team members and checking for completeness, quality assurance documentation and maintenance.

•        All other duties as assigned.

 

Knowledge, Skills and Abilities:

•        Confident in clinical functions of a nursing home. Knowledge of nursing home rules and regulations along with critical thinking skills.

•        Knowledge of a resident centered model of service delivery within neighborhood.

•        Support the mission, vision and values of Friendship Haven.

•        Abide by Friendship Haven Standards of Personal Performance and Code of Ethics.

•        Ability to work effectively with co-workers in a team environment.

•        Proficiency in writing and communicating with other team members.

•        Accountability to complete tasks with minimal supervision.

•        Ability to deliver excellent customer service and public relations.  Ability to be tactful in resolving customer service concerns and situations.

•        Attend dementia-specific training and education, as necessary.

 

Minimum Requirements:

•        Education:  One- or Two-year degree or above

•        Licensure:  Must be LPN or RN, must be willing to obtain MDS certification, if required.

•        Experience: Must demonstrate leadership qualities and initiative in primary job function

•        CPR Certification.

 

Physical Demands:

•        Sits, stands, bends, lifts and moves during work hours.

•        Willing to work beyond normal working hours, and on weekends and holidays when necessary.

 

Mental Demands:

•        Subject to frequent interruptions.

May be subject to hostile and emotionally upset residents, families and visitors.
