Resident Services Coordinator
Supervised by: Director of Social Services
Purpose of the Position
To provide a broad range of administrative assistance and support to the Simpson Health Center Neighborhoods (SHC) especially regarding the residents health information and external resident services.
Responsibilities
· Scanning, photocopying and faxing, as needed to ensure a complete medical records for all SHC residents.
· Closed medical records completion.
· Oxygen tracking and other auditing of EHR documentation as assigned.
· Track admissions and ED transfers. Provide reports as requested.
· Completes medical records requests as delegates regarding insurance appeals etc.
· Order supplies, as needed.
· Coordinator of external & internal services such as podiatry, vaccination clinics, aria dental/hearing/vision, updates and prints IRIS, responsible for part b forms/notices, managed care ins. visits etc. (also mentioned in LTC Lead position)
· Coordinates and schedules appointments and transportation for all long term care residents of the SHC.
· Tracks bed holds, NOMNCs, ABNs, payment and reimbursement forms, managed care visits and other regulatory paperwork to ensure compliance.
· Works alongside social service team and provides assistance to members of the team as delegates.
· Other duties as assigned.
Knowledge, Skills and Abilities
· Ability to deliver excellent customer service and hospitality services.
· Proficient in EHR, including but not limited to MatrixCare and EPIC.
· Attention to detail and critical thinking skills.

· Knowledge of computer/technology/phone system.

Minimum Requirements
· Education: Level of education to communicate effectively.
· Experience: Knowledge of computer software (Word, basic Excel, PowerPoint, Outlook, etc.)
Physical Demands
· Sits, stands, bends, lifts and moves during work hours.
· Should be capable of lifting up to 30 pounds as needed.
Mental Demands
· Subject to frequent interruptions.
· May be subject to hostile and emotionally upset residents, families and visitors.
