Full Time Overnight Coordinator

Purpose of the Position:

The Overnight Coordinator will be responsible for the households operating smoothly, promoting clinical and quality excellence, ensuring social needs and fiduciary responsibility to enhance the resident’s wellbeing while providing a high level of customer service. The Overnight Coordinator will collaborate with the overnight team members and residents, as well as daytime Nurse Coordinators & Leads, to determine what clinical and support services the neighborhoods need to live with faith, passion and purpose. The overnight coordinator will ensure smooth delivery of resident centered care and provide support to our overnight team members.

Household Leadership Functions:

• Provide leadership and vision to the households by modeling professionalism, customer service, collaboration, flexibility and acts as a coach to keep processes resident centered.

• General supervision of overnight clinical employees, including nurses, CMA, CNAs and other potential team members working in the households, carrying out supervisory responsibilities and auditing in accordance with the organizations policies & procedures.

• All the neighborhood direct care team members will be supervised directly by their specific neighborhood daytime coordinator, however, the overnight coordinator will work in conjunction with all daytime neighborhood coordinators to ensure smooth delivery and transparency with staff education, clinical supervision, quality assessments, skills checks while ensuring clinical and professional standards and practices are maintained and expectations met.

• Contributes to and consults on household personnel management: hiring, training and mentoring, especially of overnight team members.

• Works cooperatively and respectfully with residents, co-workers, families, visitors and

volunteers to create a warm and positive environment in the home.

• Achieve the financial objectives of the community by contributing to and accounting for the annual budgets.

• Build relationships with residents, families and guests and plays a key role in managing resident and family complaints.

• Maintain a safe, comfortable functional environment by assessing the household for sanitation and potential hazards, implementing practices utilizing disaster preparedness and emergent care, investigating and reporting all incidents and accidents, observing the team for safe work habits and addressing observations.

• Encourages a climate of continued learning and personal growth for all.

• Facilitates each household’s ability to plan and meet the clinical needs of residents and the household.

• Ensure household meetings (resident, employee and neighborhood) take place appropriately, engaging all residents and employees. Lead and participate in shift change huddles as able.

• Participates in nurse leadership “On call” rotation with SHC Nursing team, as needed.

• Coverage of overnight nurse call ins and assist with overnight caregiver call ins to find coverage or provide coverage themselves.

• Encourages extended family participation.

• Works with SHC team to implement and improve systems.

• Responsible for general oversight of nurse and caregiver’s schedules along with the completing of payroll and HR functions that may be required.

Care Plan and Documentation Functions:

• Facilitates and completes all clinical portions of the resident care plans.

• Communicates effectively with all other members of nurse leadership team to ensure nursing/clinical interventions based on identified goals are carried through and updates careplan accordingly.

• Leads investigations and reporting of all incidents and accidents during night shift and reports off to high risk and nurse leadership daily meetings.

• Audit clinical documentation to ensure proper entries by team members and checking for completeness, quality assurance documentation and maintenance.

• All other duties as assigned.

Knowledge, Skills and Abilities:

• Confident in clinical functions of a nursing home. Knowledge of nursing home rules and regulations along with critical thinking skills.

• Knowledge of a resident centered model of service delivery within neighborhood.

• Ability to work effectively with co-workers in a team environment.

• Proficiency in writing and communicating with other team members.

Minimum Requirements:

• Education: One- or Two-year degree or above

• Licensure: Must be LPN or RN.

• Experience: Must demonstrate leadership qualities and initiative in primary job function

• CPR Certification.

Physical Demands:

• Sits, stands, bends, lifts and moves during work hours.

• Willing to work beyond normal working hours, and on weekends and holidays when necessary.

Mental Demands:

• Subject to frequent interruptions.

• May be subject to hostile and emotionally upset residents, families and visitors.
