Health Service Coordinator

Supervised by: Director of Assisted and Independent Living

Purpose of the Position

  • Provide administrative and coordination support to the Assisted Living Nurse Leadership team to help ensure high-quality care that meets residents’ needs.

Responsibilities

  • Managing staff scheduling and timekeeping.

  • Supporting onboarding, orientation, education, and training processes.

  • Assisting with the coordination and tracking of related activities.

  • Maintain accurate resident information.

  • Support communication and coordination of care with families, clinics, and other appointments.

  • Assist with program compliance through record review, policy support, and other program development activities as needed.

Essential Functions

  • Create and manage staff schedules for both Gardens and River Ridge.

  • Responsible for monitoring all staff education requirements and staffing gaps.

  • Ensure bi-weekly timecards are accurate and complete approval process.

  • Coordinate new hire onboarding logistics.

  • Track orientation completion and training compliance.

  • Responsible for maintaining training records and ensuring submission to HR, when appropriate.

  • Ensure resident records are up to date and complete.

  • Scan and upload physician, orders, documents (resident and staff) in Residex.

  • Track incident reports for completion and complete non-clinical follow-up.

  • Prepare documents for reviews such as insurance, care plan, etc.

  • Assist with assisted living policy reviews and tracking.

  • Coordinate appointments, both on campus and off, along with ensuring pharmacy communications are completed correctly.

  • Manage needed supplies and non-clinical ordering, as requested.

  • Support paperwork completion.

  • Other duties as directed by your supervisor.

Knowledge, Skills, and Abilities

  • Knowledge of a resident centered model of service delivery within neighborhood.

  • Support the mission, vision and values of Friendship Haven.

  • Ability to work effectively with co-workers in a team environment.

  • Proficiency in writing and communicating with other team members.

  • Accountability to complete tasks with minimal supervision.

  • Ability to deliver excellent customer service and public relations. Ability to be tactful in resolving customer service concerns and situations.

Minimum Requirements

  • Licensure: Certified Medical Assistant preferred, but not required.

  • CPR Certification, or the ability to become certified.

  • Must complete Medication Manager course with nurse delegation to administer medications.

  • Experience: Working in aging related environments, public service, or health care preferred.

Physical Demands

  • Sits, stands, bends, lifts and moves during work hours.

  • Should be capable of lifting up to 50 pounds on a regular basis.

  • Willing to work outside normal working hours, and on weekends and holidays when necessary.

Mental Demands

  • Subject to frequent interruptions.

  • May be subject to hostile and emotionally upset residents, families, and visitors.
