Registered Physical Therapist - Full-Time
Job Title: Registered Physical TherapistReports to: Catalyst Therapy Director/Coordinator
Employees Supervised: Physical Therapy AssistantsQualifications: Bachelor’s, Master’s or Doctorate’s degree in Physical Therapy Licensed within state or under temporary licensure as a Physical Therapist
Ability to work effectively as a team member while exhibiting a positive and professional attitude
Ability to organize, plan and manage time effectively
Excellent verbal and written communication skills
Effective clinical problem solving skills
Valid driver’s license
Understanding of various payor sources and prior authorization procedures
Ability to vary schedule as needed to meet the needs of residents, including
Weekends/holidays if necessary
Ability to perform administrative functions in regards to billing for PT department, personal payroll, assisting in collection of statistics and ordering of supplies as needed
Ability to maintain patient/resident confidentiality
Ability to effectively supervise Physical Therapy Assistant(s)
Must be eligible to work in the Federal Health program
Position Summary: This individual will provide Physical Therapy services to any individual who requires skilled intervention. This includes evaluation and treatment, inservicing, consulting with other disciplines, and documentation required to efficiently meet each resident’s/patient’s needs. This individual will also provide supervision to Physical Therapy Assistant(s). These services will be provided in accordance with Friendship Haven policies and procedures, APTA guidelines, and state and federal regulations. This individual will also be able to provide inservices for nursing staff and/or other organizations on the definition and function of Physical Therapy.
Gardens Assisted Living- Part-Time Float Position
Hours: Float for the following shift: 4PM - 10:30PM
Position: The opening is for a Resident Companion who will be trained in all three Gardens Assisted Living Neighborhoods.
Job duties: This position is responsible for providing a variety of resident centered services with cross training with housekeeping for residents in their home in The Gardens Assisted Living. Assigned responsibilities include, but are not limited to: dining services including serving food, preparing coffee, after meal clean up and dishwashing, Care for resident's needs such as bathing, grooming and laundry. Assist with activity programs, as needed, escorting residents to appointments, related documentation and other tasks to meet each residents needs and preferences.
CNA or CMA is required.
