Caregiver- Journey's Neighborhood- Part-Time Weekends Only
Hours: 6:00AM - Noon
Position: Part-Time every weekend
Job duties: This position will be scheduled in the Journey's Neighborhood.
This position will provide a broad range of resident-centered care and services to meet the individual needs of each resident’s social, environmental and personal care to enhance the capacity of residents to attain or maintain their highest level of independence, offering choice and control. Prior experience working with dementia residents preferred.
Applicants must be a CNA.
Registered Physical Therapist - Full-Time
Job Title: Registered Physical Therapist
Reports to: Catalyst Therapy Director/Coordinator
Employees Supervised: Physical Therapy Assistants
Qualifications: Bachelor’s, Master’s or Doctorate’s degree in Physical Therapy Licensed within state or under temporary licensure as a Physical Therapist
Ability to work effectively as a team member while exhibiting a positive and professional attitude
Ability to organize, plan and manage time effectively
Excellent verbal and written communication skills
Effective clinical problem solving skills
Valid driver’s license
Understanding of various payor sources and prior authorization procedures
Ability to vary schedule as needed to meet the needs of residents, including
Weekends/holidays if necessary
Ability to perform administrative functions in regards to billing for PT department, personal payroll, assisting in collection of statistics and ordering of supplies as needed
Ability to maintain patient/resident confidentiality
Ability to effectively supervise Physical Therapy Assistant(s)
Must be eligible to work in the Federal Health program
Position Summary: This individual will provide Physical Therapy services to any individual who requires skilled intervention. This includes evaluation and treatment, inservicing, consulting with other disciplines, and documentation required to efficiently meet each resident’s/patient’s needs. This individual will also provide supervision to Physical Therapy Assistant(s). These services will be provided in accordance with Friendship Haven policies and procedures, APTA guidelines, and state and federal regulations. This individual will also be able to provide inservices for nursing staff and/or other organizations on the definition and function of Physical Therapy.
RN/LPN- Casual and Part-time
Hours: All Shifts including weekends and holidays
Position: Casual/PRN/Part-Time Nurse
Job duties: RN or LPN that will train in all Neighborhoods in the Health Center. This position will cover vacations and leave of absences. Possible opportunity to work a regular med pass shift (two hours in the evening) if desired once trained.
